ACTIVITIES FORMS REQUIREMENTS

To participate in LCS Athletics a student must have on file with the Activities Department:

(1) A health physical clearance, current within the last three years.

(2) A completed and signed MSHSL Annual Health and Eligibility Form.

(3) A paid activity fee.

When all materials are received by the Activities Department, the student is cleared to participate in athletics.

FINE ARTS FORMS

To participate in LCS Fine Arts a student must have on file:

(1) A completed and signed MSHSL Annual Health and Eligibility Form.

(2) A paid activity fee .

When all materials are received by the Activities Department, the student is cleared to participate in fine arts.

ATHLETIC AND FINE ARTS FEES

All Senior High Athletics – $250
All Junior High Athletics – $150
Fine Arts – $50 for Band, Choir, and 3-Act Drama

$100 for Knowledge Bowl, Speech, and 1-Act Play
