Activity Pass Form

ATHLETICS FORMS

To participate in LCS Athletics a student must have on file with the Activities Department:

(1) A health physical clearance, current within the last three years.

(2) A completed and signed MSHSL Annual Health and Eligibility Form.

(3) A paid activity fee.

When all materials are received by the Activities Department, the student is cleared to participate in athletics.

FINE ARTS FORMS
To participate in LCS Fine Arts a student must have on file:

(1) A completed and signed MSHSL Annual Health and Eligibility Form.

(2) A paid activity fee .

When all materials are received by the Activities Department, the student is cleared to participate in fine arts.

ATHLETIC AND FINE ARTS FEES

Senior High Athletics - $210
Football - $210 + $25 for equipment = $235 total fee
Junior High Athletics - $135
Football - $135 + $25 for equipment = $160 total fee
Fine Arts - $50 for each event
Band, Choir, Speech, Fall Musical, One Act Play, Knowledge Bowl

Please note that paying online will incur an $5 surchage per activity.
